We’re proud to launch our new research report, Carers’ Experiences of Hospital Discharge in Derby & Derbyshire, produced by Jackson Pountney, Hconnect Project Research Associate. This two‑phase qualitative study explores what unpaid Carers experience during hospital discharge, alongside the perspectives of healthcare professionals working within discharge pathways.

The research forms part of the wider Hconnect programme, led by Derbyshire Carers Association, which aims to improve the recognitioninvolvement, and support of unpaid Carers during hospital discharge. Through in‑depth interviews, the study highlights the behavioural, relational and systemic factors that shape Carer identificationcommunication, and inclusion in decision‑making.

These insights have informed a set of co‑produced recommendations and practical resources designed to support safer, more inclusive hospital discharge processes for Carers across Derby and Derbyshire.
