As part of Derbyshire Diverse Carers project we are asking Carers to complete a survey who identify as Black African and or Caribbean/ Mixed Ethic groups/ Asian and Minority Ethnic Communities.
Taking a few minutes to share your experiences in this survey will help us ensure that your voice is heard.
We will analyse all the responses to the survey and write a report which identifies the themes.
We will not share or publish any information which identifies individual responses but may use anonymous quotes from the answer.
