As part of Derbyshire Diverse Carers project we are asking Carers to complete a survey who identify as Black African and or Caribbean/ Mixed Ethic groups/ Asian and Minority Ethnic Communities.
Taking a few minutes to share your experiences in this survey will help us ensure that your voice is heard.
We will analyse all the responses to the survey and write a report which identifies the themes.
We will not share or publish any information which identifies individual responses but may use anonymous quotes from the answer.
This survey is for Carers who live or care for someone that resides in Derby City & Derbyshire
Survey can be found here - https://forms.gle/scXzQe5R4vxYpcANA
