Rachel Moorcroft (pictured above) is now the Carers Champion at Hartington Surgery, and recognised by her Carers Champion badge – which means if you visit the surgery or are already a patient at the surgery (and you care for a Derbyshire resident) she will be to help you with information and a listening ear regarding your caring role including:

  • Provide contact information and/or refer you to Derbyshire Carers Association
  • Register you at the surgery as a Carer
  • Update any change in circumstances regarding your caring role e.g. if you are no longer a Carer
  • Provide you with information on local support groups
  • Provide information on how to get a Carers Emergency Card
  • Signpost you to other local services and relevant support

Based at Dig Street, nr Buxton the busy surgery staff have also completed Carer Awareness training and are displaying useful resources in the Reception area and around the Surgery.

If you look after someone who relies on you, and they couldn’t manage without your support, you are a Carer.
