Adult Care Commissioning is asking DCA to capture carers views on the Derbyshire carers emergency card service.

The service is currently one of the ways in which the Council supports unpaid carers in their vital caring role by enabling carers to create an emergency plan, ahead of an actual emergency.

The details of all Carer Emergency Plans are held securely by the Council so they can be accessed quickly by Adult Social Care.

We spoke to carers in 2017 and they told us this service was important to them and suggested ways in which it could be improved.

We are not looking to end this service but are looking at ways to convert the application process and issue of the card to a computerised system. This is therefore, a really good time to seek feedback from carers again and understand how else the service could be updated and improved at the same time.

We would be grateful if you could complete the short online questionnaire about your experience of having a carers emergency card.

We will not share any information which identifies individual responses but may use anonymous quotes from feedback to highlight themes.

If you require a paper version of this survey, please email ASCH.Carers.adultcare@derbyshire.gov.uk with the your details and we will send a copy out.
