Just in Case Plan
This form will help you to plan for the unexpected and to ensure the person you care for is looked after should you become unwell or are temporarily unavailable for a period of time.

Carers Card
A Carers Card is helpful to keep on your person, in a wallet, purse or mobile phone case for example. We recommend you download, print and fill in the details alongside a 'Just in Case plan'.
If you're unable to care due to an accident or emergency, it will make whoever is helping you aware that someone relies on you for support.
That person will be able to easily identify your emergency contact and Just in Case plan location and arrange the right support for the person you care for.
