To celebrate Carers Rights Day in November, the H-Connect team hosted a coffee afternoon at the Fenton Ward, Cavendish Hospital in Buxton that week.
Tina Curran, Engagement Worker, spoke to Carers Lydia, Ann and Caroline to highlight what support Derbyshire Carers offers unpaid Carers, whilst Lesley Hodkin discussed what legal rights Carers have, as well as outlined to professionals the support that H-Connect can provide to healthcare teams.
H-Connect (Hospital Discharge Project) supports healthcare professionals to identify and support Carers early on through the hospital discharge process.
