Access to medical care: Going into hospital can be very challenging for someone with a learning disability. Did you know you can download and fill out a hospital passport for your loved one which may support them when in hospital. Some hospitals may have a Learning Disability Nurse that is available to support you whilst visiting or staying in hospital.
Carers Emergency Card: Derbyshire Carers Association are able to support you as an unpaid carer to put an emergency plan in place when you, the carer is not able to continue providing care because of an emergency. Annual Health Checks: Someone over the age of 14 and that is on the Learning Disability register at the GP surgery are entitled to annual health checks. Annual health checks are an head to toe health check checking blood pressure, weight, Height Heart and much more.
